2020年7月15日に予約していた大腸の内視鏡検査。
事情により8月12日に変更しました。
なんとか今回も無事検査は乗り切る事が出来たので，
ここに詳細を述べておきたいと思います。

これから初めての大腸内視鏡検査を行う方，
また明日や来週に内視鏡検査の予約をなさっている方，
その方の家族やその方のご友人にとって私の経験談が一助になれれば光栄です。

まず"内視鏡検査食"のレビューを行います。

8月11日(大腸内視鏡検査前の前夜），
私は6度目にして初めて"大腸内視鏡専用検査食"というものを食べてみました。

今さらなんで市販の検査食を食べたのかというと前回の内視鏡では腸の内部が
洗浄しきれずに混濁していて"スコープ"では
読み難い箇所が生じ，執刀なさる先生にもご迷惑をおかけしました。
また患者である私にとってもそれは不本意なことですので，折角やるからには
我流ではなく"検査食"を購入し食してみることにしたのです。

正直言って驚きました！！
驚いたのはその味です！
なんておいしいの？！！検査食としてでなくこのレトルトは
商品として別売するべき！！

"最後の晩餐"？！ｗ。

見た目は相当に悪いですが
ところが食べた瞬間・・?！！！

これ近所のファミレスの味なんかより遙かに敢然に越えてました！！ｗ。

私が検査する病院は国立病院で紹介された
市販する検査食はグリコ社製だったのですが，どの薬局に行ってもないｗ。
情報が古すぎて結局，"この世にはもうない商品"でした！ｗ。

でamazonで調べて，引っかかったのがキューピー社。
事情があって，7月15日の検査（前日に会社でこの検査食を食べる予定だった）
ので昼・夜用の2食用をチョイス。

結果としては私の今回の場合，
完璧に腸内をクリアにする事は難しかったのですが，
私の食べ方はイレギュラーでよくなかったと反省しています。

夕食で一気に2食分を食べてしまいました！(順番は昼・夜変わっても良いそうですが，
私にように一気に食べるのは慎んだほうが良いかもしれません）。

クラッカーがもっと多かったら，最高でした！(クラッカーは別の製造業者）
どうせ全部排出するんだからもっと多く欲しかった！

なにせ，前日から当日が終わるまでは大腸内視鏡検査の方にとって"凌ぎの日"となりますので，
この検査食のお味には企業側の患者にたいする真心="思いやり"みたいなのを感じました。

昼食は"肉じゃが"と"鮭がゆ"・・
私にとっては"うなる程のうまさ"！！ｗ。

夕食は"クリームシチュー”と少しばかりのクラッカー（苦言を述べるならこのクラッカーを
増やして欲しい！）・・どれもその味はレトルトながら逸品の部類の属しますw。

販売者はキューピー株式会社。
製造所はコープ食品株式会社。
クラッカーは前田製菓株式会社。

検査食としてはもったいない程のクォリティー！。
もっとたくさん食べたくなりますｗ。

日本の食品業者が本気を出すとえらい味になるぞという"検査"がなければこの食品は
食べる事のない代物。

さぞや"グルメ"の"求道者"？でさえ検査食までは網羅しきれず，
盲点になることでしょう。

これは特殊な内視鏡検査を控えた患者のみだけが知る"プレミアム”食となりそうです。

ただ，おいしいという側面だけでも，お勧めです！

誰もが"憂鬱"になりがちな大腸内視鏡検査・・。
味覚からしみじみと伝わってくる日本の食品メーカーの

私自身の"内視鏡検査"の話の前に
日本の内閣総理大臣(総裁）が辞任しました。

2020年8月28日、
健康問題を理由に安倍晋三首相が辞意を表明されたのです。

6月の検診では持病の"潰瘍性大腸炎"(指定難病97)の再発の兆候が確認され、
8月上旬には"再発"したことが辞意の主立った理由です。

多くの非難を受け，政治家として汚職問題も問われた安倍晋三氏。

安直に総裁を激しく非難する国民の多く(身近にも大勢いる)
は安倍首相が新型コロナウイルスの対応に追われ少なくとも"1月26日から6月20日迄"の
"147日間"もの間，公務を全力で行い休まず働いていたという事実をご存じでしょうか？

これでは"持病"は誰でも悪化します。

国家のトップもまた我々と同様に"超人"ではありませんしひとりの人間です。
あたかも強そうに見えるのはその超人的な"精神力"です。

非難する者の多くには
明白な解決策を持ってはいません。

私は"潰瘍性大腸炎"という国の難病指定であるストレス性疾患の恐ろしさをある程度は
"頭"で理解しているつもりです。しかし想像を絶するその痛みや苦しみは私の想像を
遙かに凌ぐ筈です。

長期にわたる政権運営に対し，私は国民のひとりとして
心より謝意を記し"本当にお疲れ様でした"とお伝えしたい。

是非とも今後，治療や療養に専念して頂ける事を願ってやみません。

私はIBS=irritable bowel syndrome
(過敏性腸症候群)というストレス性の疾患があります。
"ナイフで刺されるような痛みと苦闘"・・は
"潰瘍性大腸炎"ととてもよく似ているといわれます。

私自身，10年以上この疾患と闘ってきています。

医学に疎い方だと私に対して"潰瘍性大腸炎"なんですか？と
尋ねられる事がありますが，
全く異なった疾患です。

しかし私の症状としても血便(赤い血が混じった便)や下血，
粘液便(ねばねばした便)下痢，腹痛などがあり，
これらの症状が収まり(寛解)，再度ぶり返したり(再燃)を繰り返す慢性疾患として見れば
この症状は"潰瘍性大腸炎"とあまりにも似ています。

しかし私の抱えている"IBS"は"病気ではない"のです。

例えばだれでも経験があるかもしれませんが，
原因不明の突然の激しい腹痛や下痢などです。

通勤や通学の途中や試験の前や大事な会議の前や旅行先などで、
突然おなかが痛くなりトイレに駆け込んだ事はありませんか？

慌ててトイレを探す。
度重なる下痢や便秘と腹痛。
その状態が非常に緊迫した状態が
IBS"過敏性腸症候群"という疾患の典型的な症状です。

実は私は"胃と腸"にはとても自信があった人間でした。

別の記事で書きましたが"黒い犬"を飼うようになってからその"犬"から解放され
その症状が寛解して数年後に今度はIBSの疾患に悩まされる様になったのです。

また新しい"課題"が私に生じました。
疾患が生じ始めた5年間は良いドクターとも巡り会えずにその日々は"地獄"でした。

過敏性腸症候群をもう少し詳しく述べると
大腸に腫瘍や炎症など症状の原因となるような病気がないにも関わらず
腸の調子が常に悪く酷い痛みが続いたり、
便秘や下痢などの症状が数ヵ月以上にわたって続く消化管の機能障害の疾患です。

IBSは排便することで少し楽になる腹痛と下痢や便秘などの便通異常が主症状となります。

しかし排便後にすぐにまた強い腹痛が襲ってきたり，
逆に一日平穏な日になったり状態が読めない
のが患者にとってさらなるストレスとなる疾患で厄介です。

QOL(生活の質=英: quality of life)は"腎臓透析治療"をなさっている患者さんよりも
遙かに下がるとされています。(私自身にとってはこの他の疾患比較の相対評価は意味のないものです。
病気や疾患はそれぞれの患者にとって厳しい課題であり，そうした指標評価は無意味なことです）

"ストレス"が症状を悪化させる要因の1つと考えられています。
実際，内視鏡検査では腸の激しい痙攣が所見出来るケースが数多くあるそうです。

私の場合はその所見すら殆ど見当たらないそうです。
本当に厄介な人間です！！私は！！つまり検査中では痙攣が収まってしまうのです。
普段あれだけ猛烈に音とともに腸管が痙攣しているというのに！

過敏性腸症候群は症状によって4つの型に分類され"便秘型"，
"下痢型"，"下痢と便秘が同じような頻度で起きる"混合型"，
さらに"分類不能型"のタイプがあります。

症状が重症な時期は公共の交通機関(通勤電車など)に全く
乗れなくなるなど日常生活に支障を来す場合が生じてきます。

私の場合もそうでした。

私鉄電車に乗った場合（トイレのない電車）は
5分後から10分後には顔面蒼白になり，発汗と脈の乱れ，
強い吐き気が生じてきます。直腸を含め
大腸全体が下痢に向かって激しい痙攣を起こすのです。

目眩がし，立っている事が困難になります。

つまり重度のIBSの場合，目的地までの歩く道程の全てにトイレットを把握しておく必要が
生じます。途中下車して何分でトイレまで行けるか。そのトイレの
混雑状況=使用頻度はどの程度か。

駅から出た際に使えるコンビニエンス・ストアのトイレの状態はどうか。
あのトイレはおおむね何分，待たされるのか。男女共有か，別々か。

その把握に間違いがあれば，
一気に便意を催し，
耐えられない場合，
その場で排便してしまいます。

恥を忍んで"あなた"には本当の事をお伝えします。

私は社会生活で何度も何度もドアの前でこらえきれずに"脱糞"した苦渋の経験があります。
その"疾患"のたちの悪さ=QOLの質の低下はこの部分は大きいです。

公共の場で，公衆衛生の面においても，突然下痢をする他人に出くわした
際，他人はどう思うか。そんな相手を伝染病か何かと思うかもしれない。
いずれにしても忌避し，その者に声掛けするひとなど私くらいで皆無な筈です。

いつしか私は常に"お守り"として"脱糞"した際の緊急用の"キット"を
持ち歩く様になりました。

今回この記事を執筆する際に5年ぶりに中身を開封しました。
ずっと開封せずにいつも"お守り"としてビジネスバックに入れていた中身です。

この袋は10年もの間，ビジネスバックにいつも携帯していたものです。

最も厳しい時期においても私は
家の中に"引き籠もる"ことを避け（どうしょうもない時は
会社を多く欠勤した）
社会人として生活し続けました。

私は"黒い犬"が小さくなって，
その事が嬉しかったし前向きになっていたから，
この場に及んでまた自分が塞ぎ込むことが決して許せなかったのです。

"黒い犬"の経験を通じ
よほどの事がなければ動じない精神力を
身につけていました。

重度のIBS疾患は社会的生活="生活水準"を簡単に大幅に下げてしまいます。

これは公共の交通手段だけではありません。
食べたらすぐに，強い便意を催す患者も多いでしょう。
徒歩で歩いていても自動車の渋滞でも
常に"腸"の状態を監視し続けていなければならなくなります。
監視すればするほど腸は素直でストレスが加算され敏感になってゆきます。

そんな症状が重くなると恐ろしい
IBS疾患ですが"=イコール"大腸内視鏡の定期的検査"をするべきという事ではありません。

私の場合は"分類不能型"もしくは"複合型"の疾患であり，
たびたび"下血"が生じるというのが
内視鏡検査を定期的に行っている原因です。

頻繁に出血=下血がないケースであれば
私の様に短期間での内視鏡検査を薦めるドクターはいないでしょう。

従って，
腸管内部の出血から生じる"下血"=IBS（過敏性大腸症候群）のせいではありません。

つまり健常者（胃腸の良好なひと）に比較すれば
疾患からくる様々なダメージから腸そのものの本来あるべき状態と異なり，
便秘や下痢の排便の不和から生じる腸壁の異常な状態から出血が起きたり，
その炎症から腫瘍が出来易くなったりするのは健康的な?ひとに比べ遙かに多くなると
患者として考えている事です。

現実には前々回の5年前の大腸内視鏡では1.5㎝以上の腫瘍が見つかり，
検査から手術に移行しポリープの切除がされました。細胞は"生検"に送られ運良く
良性腫瘍でした。

(あの手術がなかったら今の私は執筆さえ厳しかったかもしれません。大腸のポリープは
高度なリスクで放っておけば必ずガン化します）

IBS自体は死に直結した病ではないものの，
この疾患の拗れから5-8年すると大腸癌を合併するリスクは高くなるのではと患者としては
感覚的に察しています。

そんな経緯と経験から
私の症状のケースの場合は10年の間に6回以上の大腸内視鏡検査を行った事になります。

ある意味，患者としては内視鏡検査の"ベテラン"といえるでしょう。

そんな"ベテラン"ですがw，
大腸内視鏡検査はやはり，何度経験しても辛い検査です。

これは私の体質もあるので，内視鏡検査にあたって誰もが同じ状態になるとは
限りません。IBS以外で検診で潜血反応があり，内視鏡検査をする方もいるでしょう。
便秘が酷く，腹痛で昏倒してしまうような症状の患者の方もおられるかと思います。

しかし，少しでも私の体験と経験が一助になり，お役に立てればと思い，
当日の検査前，検査中の患者としての経験を述べさせてください。

検査当日。

まず一番患者として大事な下剤を服用した腸内洗浄があります。
私の検査の場合は6回就寝する前に下剤（眼の点眼薬の様なもの）をコップ一杯の
水で飲みました。この下剤に関しては私の体質では便意を催し夜半突然目が覚めてしまうと
いう事は過去一度もありません。

今回は午前5時台に起床し，本格的な下剤の服用の支度を始めます。
下剤服用のスケージュル表の様なものがあります。一応，それに沿って下剤を飲み続けます。
その間に脱水症状やその他の理由からか大量の水を飲んでゆきます。
その量は短時間で総量3リットルです。普段の生活で短期間でこれだけの量の水分を補給する事は
まれでしょう。

私は普段から大量の飲料水を飲む事に慣れています。ビールにせよ炭酸飲料にせよ一分以内で
500ccは飲み干す事が可能なので，この量の水分を短時間で飲みきる事自体はさほど苦痛ではありませんが，
普段そうした大量の水分を飲む事のない女性や老人，
その他の男性にとってこの作業はもの凄く苦痛な作業となる筈です。
慣れていない方は嘔吐してしまい，検査が出来ない方も大勢います。

このあたりが乱暴な検査なのですが，年齢や体重に合せた飲み方というものは
ありません。大柄な男性も小柄な女性も3リットルを飲み干す必要があるのです。
検査中の苦痛よりもこの"支度"で患者の苦痛はMAXになってしまい，
検査に耐えられずにつまづいてしまうケースがあります。
いずれにせよ腸管内部に便を残すことは
折角のスコープに依る検査の精査を低下させ台無しにしてしまいます。

これは私の場合ですがいつも
大量の下剤を飲み始めて，一時間以内は何の変化も起きません。
普段通りです。しかし一時間を経過し始めたあたりから突然猛烈に下剤の
人工的な激しい腹痛が生じてトイレに駆け込みます。

それからが勝負となります。
顔面が蒼白になるのがわかり，脈がおかしくなるのが理解出来ます。
悪心が生じ，冷や汗を掻きます。しかしその不安になって検査を中止する事は
私の場合は一度もありません。

これは私があくまで”自己責任"で判断している事なので
あなたが同じ症状になって大丈夫であるという保証は出来ません。

中止するかしないかは，それぞれ患者さん次第で各自，医療機関に問い合わせるか
一旦中止し検査予約している病院にて判断して貰ってください。

そのあとは排尿しているのと違いのないような感じで
下剤が常に排出され腸内の細かい排泄物を除去してゆきます。
その回数はひとに依るかもしれませんが，トイレにゆく回数は数十回だと思います。

体力的にもろくに昨夜から当日も食事も摂取しておりませんので，
疲労感を感じます。便意が生じるまではベットに横になるか床に寝転がるか，
喫煙者であればおおいに煙草を吸うなり，楽にしている事が大事です。

次第に，強い腹痛も便意も収まってゆく筈です。

私の場合は，禁忌とされる自動車に依る自走で病院まで検査しに行っています。
自宅からの距離は数十㎞も先ですので車の方が都合が良い為です。
本来は自転車の運転も禁止です。筋肉を弛緩する注射や安定剤の注射を打つ事も
ありその影響は数時間はあります。
本来検査当日は，判断力も体力，精神力も下がっている事が多いので
絶対におやめください。私はあくまで自己責任で行っているにしか過ぎません。

また，検査当日の下剤服用ですが，それを前倒しして前日(前夜)に行う方がいます。
私は過去行った事はありませんが，この行為もリスクは高いのでおやめください。
就寝中に脱水から生じた身体のトラブルは，重大事故に繋がる危険性があります。
体力のある方も人間の身体というのは体調というものがあります。
早めの支度は何事にも良いというのは，この検査においては当て嵌まりません。

また，初めての検査の方や，
不安のある方，高齢者の方などは家族など
患者ひとりでなく，同伴者のもとで下剤を掛けてゆくのがベストです。

以上，やや乱暴な筆致ではなはだ恐縮しておりますが，
大腸内視鏡検査は胃カメラの検査よりも遙かに難易度(患者負担)が高い検査です。

Moreover, because it is the patient it unwilling for me, not one's own way because it does with much trouble but "Inspection food" decided to be bought and to eat.
To tell the truth, I was surprised.

Quality of too good as diet for examination.

This food is an article without the eaten thing if there is no "Inspection" that it will become a great taste when the food agency in Japan puts out seriousness.
Even the diet for examination is that "Those who seek after truth" ? of "Gourmet" not to be able to finish being being surely covered, and that becomes a blind spot.
This seems to become "Premium" food that only
the patient who refrained from special endoscopy learns.

Ahead of the story of my own "Endoscopy"

Shinzo Abe who received great deal of blame, and was asked graft problem as politician.
Does Prime Minister Abe know the fact of being chased, at least doing one's best official duties between "147 days" one of "June 20 from January 26", not taking a rest, and having worked to the correspondence of the new Coronavirus virus as for many of (There are many people near themselves) people who intensely cheaply criticize the president?

With this, "Chronic disease" deteriorates everyone.
The top of the nation is not "Superman" as well as us and one man, too.
It is like the superhuman "Willpower" to look strong
It is not with a clear solution in many of people who criticize.

I will understand fear of a stress disease that is the intractable disease in the country "Ulcerative colitis" specification from "Head" to some degree.
However, the pain and suffering that outs of all knowledge are sure to surpass my imagination far.
It wants sincerely to record gratitude for a long series of political management as my.. one of the people and to tell, "..truth.. thank you very much".
Being wish the thing that it is possible to concentrate on treatment and recuperation by all means in the future doesn't stop.

I have a stress named IBS=irritable bowel syndrome (irritable bowel syndrome) disease.

For instance, it is sudden severe abdominal pains and diarrhea etc.
of an uncertain cause though there might be every experience.
Do before on the way and the examination of commuting and going to school, before an important conference, and the destination, etc. suddenly make to having a pain the one's inside and isn't there running in in the rest room?
It searches for the rest room panicking.
Diarrhea, constipation, and stomachache that happens one after another.
The state that the state becomes tense very much is a typical symptom of
the disease IBS "Irritable bowel syndrome".

I was actually man that "Stomach and intestines" was very confident.
The symptom was liberated from the "Dog" after it came to keep "Black dog" and came the remission and to be going to be annoyed this time by
the disease of IBS in several years though wrote by another article.
Moreover, new "Problem" was caused in me.
It came round with good doctor, it was not possible to meet for five years when it had begun to cause disease, and "Hell" the every day
